What documents are required for health code application?Ģ.1 Required documents prior to travel (please only take the test after 14 days you have been fully vaccinated for COVID-19, exclude booster dose):ġ) For people who have fully vaccinated with inactivated COVID-19 vaccine, please provide two negative COVID-19 PCR test reports Ģ) For people who have fully vaccinated with mRNA COVID-19 vaccine, please provide a negative COVID PCR test report and a negative IgM (Nucleocapsid) serology antibody test report ģ) For people who have not been vaccinated, or not been fully vaccinated against COVID-19, please provide a negative COVID PCR test report and a negative IgM (Nucleocapsid) serology antibody test report īeing fully vaccinated means that people have received all required doses of inactivated vaccine or mRNA vaccine within the recommended gap between doses, and the last dose being at least 14 days. In case of the certificate could not be obtained through the Wechat app, please provide other forms of the vaccination certificate issued by a Chinese government official, and complete the Letter of Commitment on COVID-19 Vaccination (refer to Appendix 1, Chinese nationals must sign the letter with Chinese characters).Ģ) People who have been vaccinated in New Zealand, please provide the International Travel Vaccination Certificate (refer to the pictures below) which can be obtained from the website of New Zealand Ministry of Health. Alternatively, people may request an immunization status from their GP with the following information: full name, date of birth, name of vaccine, date of the first and second doses were given, and place of given. It also must be signed and stamped by GP or clinic.
